If your doctor or dentist does not order the device for you directly, and bill your insurance company themselves, there is another way to seek reimbursement. You can purchase this item yourself, and submit to your insurance company the following:

1. A prescription from the Dentist / Doctor
2. A copy of the sales receipt.

If your order is contingent on getting reimbursed, then we highly recommend confirming this with your insurance company prior to making the purchase, to ensure that you will receive reimbursement.
